Dear Community Healthcare Center Patrons,

Here at Community Healthcare Center we are dedicated to protecting our patients’ privacy and confidentiality. We value your trust and thank you for your patronage. Unfortunately, we are here to disclose that some patient information may have unfortunately been lost. We regret to inform you that an electronic file or USB drive was lost/stolen.  Despite rigorous efforts to safely record health information, our internal investigation suggests that a potential breach of patient information may have occurred. This USB contained names of clinic patients and their referral information which includes names of referral doctors. The USB may have also contained hospital and nursing home face sheets. These facesheets may contain patient names, addresses, insurance information, family contact information, admitting diagnosis, date of birth and SSN#. Those affected by this incident are patients for which referrals were requested in our clinic and those who I provided medical services in the hospital or nursing homes from June 2016 to April 2017. We are utilizing all available resources to recover any potentially lost information and will continue to investigate the cause of this incident and how to prevent future occurrence. We will update this website as new information becomes available.

As technology advances, so do healthcare systems and their dynamics. In response to this incident, we are undergoing a full conversion from paper medical records to an electronic medical record (EMR). Although EMR use is now the standard of the healthcare field, there are some inherent risks with this method. Nevertheless, for progress, security, and improvement, we will digitize traditional paper medical information for a more efficient and accurate workflow. With the medical team able to access your information and resources faster and more accurately—better for you and your health. We have spared no effort in maximizing our cyber-security to ensure the safety of our patients’ information and to ensure compliance with the Health Information Portability and Accountability Act (HIPPA). We apologize for any inconvenience related to this incident. Please do not hesitate to relay any questions regarding this unforeseen incident or any patient privacy and confidentiality concerns.
